    Rapamycin(CAS NO.53123-88-9) is an immunosuppressant drug used to prevent rejection in organ transplantation, it is especially useful in kidney transplants. Rapamycin dose is important in clinical. 

    High immunologic risk
    Maintenance: 5 mg PO 
    Dose adjustments: trough concentrations 10-15 mg/mL until week 52 

    Low-to-moderate immunologic risk
    Load: 6 mg PO
    Load: Up to 15 mg PO
    Maintenance: 2 mg PO qDay
    <40 kg: load 3 mg/sq.meter PO, maint 1 mg/sq.meter PO qDay
    Adjust dose according to serum concentration: 16-24 ng/mL x1 year then 12-20 ng/mL thereafter
    May discontinue cyclosporine gradually 2-4 months after transplant in patients with low immunologic risk, & increase dose of sirolimus
